    Two stimulation procedures, one epidermal and one dermal, with distinct depths, mechanisms and biologic responses that determine which clients benefit from each. MICROCHANNELING is an epidermal focused procedure that creates uniform microcolumns across the stratum corneum and viable epidermis using fixed depth, vertically delivered microprojections. Because most devices remain within the epidermis and do not meaningfully enter the dermis, microchanneling does not function as collagen induction therapy. Instead, its primary role is to enhance epidermal turnover and increase the penetration of topical actives such as growth factors, peptides and barrier supportive formulations. The biologic response is superficial and non inflammatory, making it appropriate for early textural changes, mild pigment irregularities and clients who require minimal downtime. It is also well suited for sensitive or vascular prone skin because it avoids oscillation, drag and deeper dermal trauma. Contraindications include active infection, open lesions, uncontrolled acne, recent sunburn, tretinoin use and impaired wound healing. Although generally well tolerated, microchanneling still requires a stable barrier and should not be performed on inflamed or sensitized skin. Microchanneling is selected when the goal is controlled epidermal stimulation, enhanced topical delivery and minimal inflammation. MICRONEEDLING is a true dermal remodeling procedure that uses oscillating needles to penetrate beyond the epidermis into the papillary and, when indicated, the reticular dermis. This controlled mechanical injury initiates a wound healing cascade involving inflammation, fibroblast activation and collagen reorganization. Because of its depth and biologic response, microneedling is appropriate for acne scarring, etched lines, deeper wrinkles, laxity and advanced textural changes that require structural change rather than superficial stimulation. Depth can be adjusted to target specific dermal layers, allowing individualized treatment based on scar depth, skin thickness and clinical goals. Contraindications include active infection, open lesions, uncontrolled acne, recent sunburn, tretinoin use, keloid history, impaired wound healing and inflammatory skin disorders. Additional caution is required for clients prone to post inflammatory hyperpigmentation or those with thin or fragile skin. Microneedling is selected when the goal is structural dermal change, including scar remodeling, collagen reorganization and improvement of deeper wrinkles or laxity. Understanding the biologic response at each depth ensures appropriate treatment selection and reduces the risk of adverse outcomes. The choice depends on the client’s skin thickness, inflammatory tolerance, barrier stability and the degree of remodeling required.

    Beyond the Powerhouse: PhotoBioModulation’s  Network Revolution in Regenerative Medicine I have seen some recent posts, and I realize many of them have missed the mark regarding PhotoBioModulation. I decided to write an article about this. It’s more than just cytochrome C oxidase. Photobiomodulation (PBM) has evolved beyond the traditional focus on cytochrome c oxidase (CCO) as its primary mechanism, emerging instead as a sophisticated, network-level signaling therapy that orchestrates cellular responses across multiple biological systems. While CCO remains an important chromophore, modern research shows that red and near-infrared light interact with a broader range of targets, including membrane receptors, ion channels, nitric oxide (NO) stores, and cAMP pathways, leading to coordinated changes in redox balance, calcium flux, and bioelectric signaling. These primary signals influence major transcriptional and kinase pathways—Nrf2, NF-κB, PI3K/AKT, MAPK, and CREB—that regulate inflammation resolution, cell survival, tissue perfusion, and regeneration. Wavelength selection is crucial: red light (600-700 nm) best affects superficial tissues for wound healing and skin rejuvenation, while NIR (800-1100 nm) penetrates deeper tissues for applications in bone, neural, and joint health. PBM's effects also include immune modulation (shifting macrophages toward pro-resolution phenotypes), mitochondrial dynamics and intercellular transfer, stem cell activation, extracellular matrix remodeling, and the suppression of inflammatory signaling associated with aging. Timing through pulsed delivery creates biological rhythms that enhance signaling without causing oxidative stress, adding another layer beyond simple photon absorption. Clinically, PBM acts as a powerful enhancer of regenerative therapies—working synergistically with PRP, exosomes, mesenchymal stem cells, hyperbaric oxygen, and systemic treatments by creating an optimal cellular environment primed for repair.     #Scientists just made 50-year-old skin cells behave like they’re 20 again. Scientists at The Babraham Institute in #Cambridge have found a way to make old #human skin cells behave as though they are about 30 years younger, without turning them into #stemcells. Using a modified version of a #Nobel #Prize–winning stem #cell #technique, they briefly exposed adult #skin #cells (fibroblasts) to a set of #reprogramming #molecules known as #Yamanaka factors, but stopped the process early. After 13 days, the cells had shed many #molecular signs of #aging yet were still able to regain their identity as #skin cells. Tests showed that these “#rejuvenated” cells more closely matched the #biological profile of much younger #cells when #scientists looked at #chemical tags on #DNA (the #epigenetic clock) and patterns of #gene activity (the #transcriptome). Crucially, the younger-looking #cells also acted younger. The rejuvenated #fibroblasts produced more #collagen, a key #protein that helps skin stay firm and heal after injury, and they moved more quickly to close an artificial “wound” in a lab dish than untreated older cells. The #researchers also saw age-related changes reversing in #genes linked to conditions like #Alzheimer’s #disease and cataracts, hinting at wider #medical potential. Although this work is still at an early stage and the underlying #mechanisms are not fully understood, it suggests that in the future #scientists may be able to selectively refresh aging #cells to improve tissue repair and possibly delay some effects of age-related disease, without completely resetting cells to a stem #cell state.     A 2015 study (PMID: 25431878) in Rejuvenation Research showed that aerobic exercise reversed [molecular signatures of] every hallmark of aging at the cellular level (given the mechanistic nature of this study, it was an animal model). After just 5 months of treadmill running, the intervention improved mitochondrial integrity, stem cell function, proteostasis, DNA repair, and reduced systemic inflammation (suggesting that exercise acts on nearly every major aging pathway at the cellular level). Outcomes: - Extended lifespan and improved physical performance - Reversal of mitochondrial DNA instability and improved respiratory chain function - Reduced accumulation of damaged proteins and enhanced antioxidant defense - Restoration of satellite cell numbers in skeletal muscle - Attenuation of chronic inflammation and cellular senescence markers These findings align with the nine hallmarks of aging described by López-Otín et al., 2013 and suggest that physical activity may exert systems-level anti-aging effects, at least in accelerated aging models. Translational Perspective: While this study used a genetically modified mouse model of progeria, its findings support a large body of epidemiological and human data linking regular physical activity to delayed biological aging, reduced all-cause mortality, and preserved muscle and cognitive function in older adults. Human studies have also shown that endurance and resistance training can: - Improve mitochondrial biogenesis (via PGC-1α) - Enhance stem cell recruitment in muscle and brain - Reduce inflammation and oxidative damage - Support insulin sensitivity and vascular health However, humans age through multifactorial and heterogeneous processes, not all of which are captured by animal models like the one used in the referenced study. Exercise may attenuate, but not reverse, certain aging trajectories, and effects likely depend on dose, duration, genetic background, and baseline health.
