    Data reveals real 90-day change patterns. I often see people start a transformation journey expecting a straight line up and to the right. They anticipate that motivation will compound linearly, that habits will stack neatly, and that they will feel better with every passing week. My clinical observations suggest a very different reality. When I map what actually happens in the first three months of genuine behaviour change, the trajectory is rarely smooth. It follows a specific, predictable pattern of resistance and recalibration that I call PREP™. Understanding this timeline prevents you from interpreting normal friction as failure. Here is what the first 90 days really look like. Weeks 1–3 are about Prevention. We stop the slide before we try to fix the problem. The goal here isn't optimisation. It is stabilisation. You remove pressure rather than adding habits. Sleep steadies. Guilt reduces. Interestingly, motivation often dips during this phase because the adrenaline of self-criticism switches off. That is not a warning sign. It is a sign of safety. Weeks 4–6 bring the Rewire phase. This is where resistance appears. Old patterns push back hard. One disrupted week can trigger the belief that "this isn't working." This is simply identity lag. Your nervous system is recalibrating to a new normal. It feels messy and uncomfortable, but it is the necessary work of changing how you operate, not just what you do. Weeks 7–9 are for Elevation. Subtle wins emerge. Energy becomes predictable. Decisions feel lighter. You notice you pause instead of reacting. Nothing dramatic happens, but life feels less brittle. Finally, Weeks 10–12 are where you Perform. Trust replaces intensity. You recover faster from off-days. Habits feel like defaults rather than forced efforts. Capacity is higher, making consistency easier. Real change is messy, non-linear, and quiet at first. It works when you fix the conditions before demanding performance. If you are in the messy middle right now, remember that resistance is part of the architecture, not a sign of collapse.

    What changes behaviour? It's not what many of us try. We often try to: - Increase knowledge - Change beliefs about consequences - Shape general attitudes - Improve general skills - Sanction certain behaviours But those are not the best strategies. According to a large synthesis of 147 meta-analyses done by Albarracín et al. (2024), the more effective strategies are to: - Provide access to resources that facilitate the behaviour - Establish habits that make the behaviour routine - Provide help with performing the behaviour - Shift attitudes about the behaviour itself - Help people feel they'll fit in by doing the behaviour - Teach skills to execute the behaviour Material incentives can be effective, but the tend to undermine intrinsic motivation to do the behaviour. Sometimes, that doesn't matter, but many times it does. That's why I've not included it in these top six high-impact targets for intervention. Wellbeing and performance are closely connected to behaviours, so to enable flourishing, as individuals or in teams, we need more than just knowledge. We need to change behaviours.     🔬 New evidence on how small lifestyle changes could add years to your life A new UK Biobank study followed 59,000 adults to find the minimum combined improvements in sleep, physical activity, and diet needed for meaningful gains in lifespan and healthspan. Key findings: → +1 year of life: Just 5 extra minutes of sleep, 2 minutes more physical activity, and half a serving of vegetables daily → +4 years disease-free: 24 minutes more sleep, 4 minutes more activity, plus modest diet improvements (one cup of vegetables, one serving of whole grains, two servings of fish weekly) → Optimal behaviors (7-8 hours sleep, 42+ min daily activity, quality diet) were linked to ~9.4 additional years of both lifespan and healthy years Practical takeaway: Small, simultaneous changes across sleep, movement, and nutrition may be more effective and sustainable than dramatic changes in any single area.     The original 4 pillars of longevity are not “biohacks.” They’re the metabolic syndrome antidote. If you want one framework that actually moves outcomes, it’s this: 1) Movement (fitness + strength) 2) Nutrition (metabolic stability) 3) Sleep (hormonal + glycemic control) 4) Stress & connection (neuroendocrine + immune tone) Why do these matter so much? Because metabolic syndrome is basically the clinical “receipt” for when these pillars break down: central adiposity + insulin resistance + dyslipidemia + hypertension → accelerated vascular aging, cognitive risk, and shortened healthspan. Pillar 1: Movement — the capacity pillar What it impacts: insulin sensitivity, visceral fat, blood pressure, triglycerides, HDL, inflammation. How to test it (simple → advanced): • Resting BP, resting HR • Grip strength / sit-to-stand • VO₂ max (gold standard) What to do: • Zone 2 base + intervals + resistance training (minimum effective dose beats “all-or-nothing”). Pillar 3: Sleep — the endocrine reset What it impacts: cortisol rhythm, appetite hormones, glucose control, blood pressure. How to test it: • Sleep duration + regularity (7–9 hours is a useful target) • Screen for sleep apnea if snoring/daytime fatigue/HTN • Wearable trends (useful, not perfect) What to do: • Protect a consistent sleep window • Morning light + evening dark • Treat apnea aggressively when present Short sleep is repeatedly associated with higher metabolic syndrome risk. Pillar 4: Stress & connection — the “invisible biology” What it impacts: inflammation, cardiometabolic risk, adherence, recovery capacity.     If knowledge changed behavior, every health app would work. People know they should exercise. They know they should sleep more. They know they should save money, take medication, practice a skill, eat better, stretch, hydrate, meditate, and go outside. And yet… most people still struggle to turn intention into action. That’s why I loved this recent review in Nature Reviews Psychology (https://lnkd.in/gnkr2P_6), which synthesized meta-analyses across behavior-change domains: health, environment, social behavior, and more. 💡 The big takeaway: the weakest behavior-change interventions tend to target what people know or believe. The strongest ones help people actually do the behavior. That distinction matters for every consumer app trying to improve people’s lives. Too many products are built as if the core user problem is a knowledge gap: 🧠 “Here’s why this matters.” 🧠 “Here’s what you should do.” 🧠 “Here’s the science.” But the better product question is: What makes the desired behavior easier to start, repeat, and recover? The review points to more powerful levers: ✅ increasing access ✅ building habits ✅ providing social support ✅ using reminders and monitors ✅ shaping norms ✅ practicing specific behavioral skills That’s very aligned with how we think about learning at Duolingo. We don’t just tell learners that daily practice matters. We design around the behavior: 🟢 bite-sized lessons 🟢 streaks 🟢 reminders 🟢 immediate feedback 🟢 social motivation 🟢 accessible practice from anywhere The goal is not simply to make someone value learning. It’s to make learning easier to begin, easier to repeat, and easier to come back to after life gets in the way. For consumer apps -- especially in health, wellness, education, and finance -- this is the real design challenge. Don’t just educate users into wanting change. Design the environment that helps them become the kind of person who changes.

  • We keep telling cancer patients to "make lifestyle changes," then act surprised when nothing changes. It's not them. It's us. When a patient leaves with six pillars to fix — nutrition, movement, sleep, stress, connection, substances — they don't pick the most important one. They pick the easiest one, or none. Not because they're unmotivated. Because we handed them a to-do list and called it a care plan. Here's the reframe most of us never got: this is an information problem, not a willpower problem. We assume the patient knows what "eat better" means and just isn't doing it. Usually they don't know — not with enough precision to act on by Tuesday. And "I didn't know" looks identical to "I didn't try" from the outside. So we read an information gap as a motivation failure, double down on the lecture, and watch it fail again. Behavior change doesn't work by volume. It works by leverage. Pick the one or two moves that move the most — then make them specific enough to schedule. In oncology right now, with the GLP-1 wave stripping lean mass off patients, the two highest-leverage levers are obvious: resistance training and protein. So don't say "stay active." Say: "Lift twice a week. 30 grams of protein at every meal." The 2-minute move, providers: Before your next patient walks out, cut the list to one thing. Name it with a number. One specific, schedulable action beats a perfect plan they'll never start. Stop prescribing effort. Gradually increase the difficulty as the habit becomes ingrained. Consistency is key – repetition strengthens the habit loop. 🚫 Break Unwanted Habits: Reduce motivation: Reflect on the negative consequences. Decrease ability: Make the habit harder to perform. Eliminate prompts: Remove triggers from your environment. Example: Want to read more? 📚 Motivation: Remind yourself of the knowledge and relaxation reading brings. Ability: Start with just 5 minutes a day or a few pages. Prompt: Place a book on your pillow to remind you to read before bed.     Motivation is the least reliable driver of behaviour change. It fluctuates. A good morning, a bad week, a stressful month, motivation moves with all of it. And most people experience this. They wanted something, started, fell off, and concluded they didn't want it enough. Wendy Wood, a psychologist at USC, studied this for 30 years. She found that the people who changed most durably were the ones who changed their environment until the behaviour became automatic. It had nothing to do with how motivated they were. The gym on the commute home. The kitchen with healthy food in it. Small contextual shifts that made the right behaviour easier to do than avoid. 43% of what people do every day isn't a decision. It's a context firing. Same place, same time, same cues, the brain automatically starts executing. Once a behaviour crosses that threshold, motivation becomes largely irrelevant. The context carries it. The people who relied on motivation alone kept cycling. High intention, inconsistent follow-through, same conclusion every time. The ones who lasted stopped trying to sustain motivation and started building situations where they won't need to rely on it.
