    I once spent 6 months negotiating a final account as a graduate QS. £2.3M project, £400K in disputed variations. The other QS had better records. We settled for £𝟭𝟴𝟬𝗞. Here's what I learned about documentation: The client's QS walked into the meeting with a folder thick as a phone book. Every variation referenced. Every delay photographed. Every instruction timestamped. I had... Excel spreadsheets and some email chains. The painful reality: We both did the same work. We both managed the same changes. But only one of us could prove it. What separated their approach from mine: They built the claim file during the project, not after it. While I was updating cost reports at month-end, they were capturing evidence daily. When negotiation time came, they didn't need to "build a case" - they just opened the file. The lesson that cost me £220K but taught me that:  1. Documentation isn't about compliance. It's about commercial protection.  2. Every day you don't capture what happened is a day you can't defend what you're owed.  3. Final accounts aren't won in the negotiation room.     Data Integrity is not a documentation activity. It is the foundation of every GMP decision. In pharmaceutical operations, every batch release, OOS investigation, stability conclusion, validation decision, regulatory submission, and Quality judgement depends on one simple question: Can we trust the data? Data Integrity means data must remain: ✅ Attributable – who did it, when, and why ✅ Legible – readable and understandable ✅ Contemporaneous – recorded at the time of activity ✅ Original – first capture or verified true copy ✅ Accurate – scientifically correct and truthful ✅ Complete – including raw data, metadata, repeats, re-runs and audit trails ✅ Consistent – logical sequence of events and timestamps ✅ Enduring – protected throughout the retention period ✅ Available – retrievable for review, investigation and inspection But in real GMP environments, Data Integrity goes far beyond ALCOA+. It includes: 🔹 validated computerized systems 🔹 controlled access and unique user IDs 🔹 audit trail review 🔹 secure backup and archival 🔹 controlled spreadsheets 🔹 good documentation practices 🔹 traceable laboratory raw data 🔹 investigation of missing, altered, repeated or unexplained data 🔹 risk-based governance 🔹 training and accountability 🔹 leadership-driven quality culture A strong Data Integrity program is built on three pillars: People – trained, ethical, accountable users Process – controlled SOPs, reviews, investigations and CAPA Technology – validated, secure, traceable systems Weak data integrity does not only create inspection risk. It creates decision risk. Because when data is incomplete, manipulated, poorly reviewed, overwritten, backdated, selectively reported or not traceable, the organization may lose confidence in: ❌ test results ❌ batch release decisions ❌ stability trends ❌ method validation conclusions ❌ deviation investigations ❌ product quality assurance The real maturity of a GMP organization is visible in how it handles data when nobody is watching.     Documentation is not paperwork. It is a tool for learning, reflection, and professional growth. Working in the development sector has taught me one thing very strongly: if we don’t document our work, we may remember the experience—but we may lose the learning. Over time, I have made documentation an integral part of my journey. Whether it is a field visit, a stakeholder meeting, a challenge faced, a successful intervention, a training session, or simply a reflection from the day, I try to document it. I regularly document: 🔹 My Learnings – What did I learn today that I didn’t know yesterday? 🔹 My Challenges – What didn’t work? What barriers did I face? What could I have done differently? 🔹 My Field Visits – What did I observe? What did stakeholders say? What patterns did I notice? 🔹 My Reflections – How did the experience change my understanding of the problem, people, or system? 🔹 My Progress – What has changed over a week, month, or a longer period? I have found that documenting creates a powerful repository of experiences. A daily reflection helps me capture the small but important observations. A weekly reflection helps me identify patterns and connect different experiences. A monthly reflection helps me step back and ask: What impact am I creating? What am I learning? Where am I growing? And what needs to change? In this sector, where our work often involves complex systems, communities, government stakeholders, field realities, and constantly evolving challenges, documentation becomes even more important. It helps us move from: Experience → Reflection → Learning → Action → Improvement And good documentation doesn’t have to be complicated. Some tools that can make it easier: 📝 Google Docs / Microsoft OneNote – for detailed notes, reflections and field observations 📊 Google Sheets / Excel– for tracking activities, data, progress and follow-ups 🗂️ Notion – for creating a structured personal knowledge and learning repository 📱 Google Keep / Notes – for capturing quick observations and ideas on the go 🎨 Canva – for converting learnings and field insights into visually engaging stories 🧠 Miro – for mapping ideas, reflections, stakeholder journeys and systems 🤖 AI tools– for organising raw notes, identifying patterns, summarising reflections and turning observations into structured documentation For me, documentation has gradually evolved from “writing what I did” to “understanding why I did it, what happened, what I learned, and what I can do better next time.” And perhaps that is the real power. Your work creates experiences. Your documentation creates learning. Your reflection creates growth.     An auditor walks into a bar and says — “Show me the procedure for pouring that drink.” It’s funny. And uncomfortably familiar. In GMP, documentation is non-negotiable. If it’s not documented, it didn’t happen. But the real question isn’t whether to document, it’s where to stop. On one end: ➤ A one-page batch record ➤ One signature at the end ➤ Compliant? Maybe. Defensible when something goes wrong? Probably not. On the other: ➤ A record of every click, movement, and “supporting” step ➤ Massive. Exhaustive. ➤ Still unclear what actually mattered. Both extremes miss the point. Documentation isn’t just proof that work happened. It’s evidence that the process was under control. That balance usually comes from three deliberate steps: 1️⃣ Start with a risk assessment 2️⃣ Define a clear control strategy 3️⃣ Document how each control was executed and verified Not every step deserves the same level of detail. But anything that protects product quality, patient safety, or data integrity does. Minimal records may check the compliance box, but they often fail when it’s time to: • release a batch with confidence • investigate a deviation • understand why something happened, not just that it did That’s why documentation remains a persistent regulatory pain point, and why automation and system integration are raising the bar even higher. Regulators don’t want more documentation. They want the right evidence, at the right level, for the right risks. So maybe the better question isn’t: “How much should we document?” It’s: What evidence would we want if this drink didn’t taste right tomorrow? Where have you seen teams strike the right balance?
